Well communication is key. Set up a system of feedback. Rate pain on a scale of 1-10, anything over 7 make sure you mention it. Yes, you can probably take quite a bit of pain, but it's not really a situation of "no pain, no gain."
You can still do beneficial work for someone without leaving them with bruises. And even deep work can be relaxing. With your health issues I would definitely want to check on your breathing muscles. Serratus Anterior, Serratus Posterior, Scalenes (anterior and posterior), the Quadratus Lumborum and the diaphragm for a start. Gentle work in these areas can be really helpful to those who experience a lot of stress & anxiety.
